Your answers live only in this browser tab while you're filling out the questionnaire. We don't have a database storing your responses, and nothing is sent to a server as you type.
When you finish, the PDF — both the written narrative and the structured medical note — is built right in your browser and saved to your device. We don't email it, upload it, or keep a copy.
Once the PDF is on your device, it's yours. Share it with providers as you see fit, and store it somewhere you trust (a password-protected folder, a secure cloud drive, or encrypted storage on your phone).
HIPAA applies to healthcare providers, health plans, clearinghouses, and the business associates that handle protected health information on their behalf. This tool is designed for personal and family use — it does not store your information and does not transmit it to any provider.
